    SCHOLARSHIP APPLICATION

    PURPOSE:

    To financially assist Houston area upper class students attending Grambling State University.

    The funds will be presented in the form of a scholarship for Houston area students distributed by the financial aid office and applied to the students account..

    ELIGIBILITY: You must be:

    1. A resident of Houston or surrounding areas. (optional)

    2. G. P. A. of 3.0 or above, or can be based on need if less than a 3.0 G.P.A but no lower than a 2.3 G.P.A. (optional)

    3. A sophomore and above attending Grambling State University.

    REQUIREMENTS:

    1. Complete an application.

    2. Include one letter of recommendation from your Dean or professor.

    3. Submit most recent official transcript from Grambling.

    4. Proof of need provide financial aid award letter.

    5. Include an essay no more than two pages.

    6. A wallet size photo attached to the application.

    7. Mail completed application to:

    No applications postmarked after that date will be accepted.

    Please prepare a concisely written composition not less than two page addressing on of the topics below.

    Note: Remember to be creative in presentation, style, and writing technique. Essays will also be judged on grammatical structure. Essay should be in 12 pitch, times new roman and double spaced.

    • I am a Houston area student who is attending Grambling State University. (optional)

    • I will use the proceeds of any scholarship aid received for the payment of required tuition, housing, materials/books. (optional)

    • This application, including the essay is my own work or formally cited for other sources. I affirm that the information contained herein, is0 true and accurate to the best of my knowledge.

    • I agree to release an official transcript of my grades to the DLW Scholarship & Charitable Foundation, Inc.

    • If I am awarded a scholarship, I will provide satisfactory evidence as required by Foundation of my enrollment in school during the period for which the scholarship is awarded, a photo of myself, and a testimonial.

    • I understand that submission of this application constitutes permission to use my name and/or photograph for promotional purposes.

    • Attend the annual DLW Scholarship & Charitable Foundation Black & Gold Scholarship Gala. Complete 10 volunteer hours in the Houston area with an organization designated by the Foundation to be completed during the period for which the scholarship is awarded.
